Why does the bathroom smell of sewer only some days?

A trap has gone dry, or a vent is letting the seal get pulled. Guest bathrooms and floor drains are the usual ones, because nothing runs through them for weeks. Running water into the fixture refills the trap. If the smell comes back within days with normal use, the venting is the part to have looked at.

Is a valve under the sink the same as a real vent?

An air admittance valve is a substitute, not the same thing. It lets air in but never lets gas out, and it wears out silently after some years. Kitchen islands and added bathrooms often depend on one. If a cabinet smells and the trap holds water, that valve is the first suspect.

Does cast iron drain pipe wear out?

It does, from the inside. Waste sits along the bottom of a horizontal run and eats the metal there first, so the pipe thins along the base while the top still looks fine. That is why an old stack can look solid and still be close to failing.

Can old drain pipe be relined instead of dug up?

Sometimes. Lining works where the host pipe still holds its shape and the run is reachable from both ends. Where iron has collapsed or the slope is wrong, it has to come out. A plumber inspects first, because the answer depends on the pipe rather than on preference.
